If you are already a Kustom Signals Preferred Kustomer or Preferred Authorized Service Center, please login with your user id and password to automatically enter your contact information. Once you have completed and submitted the form, your return material authorization number will automatically be assigned.
If you are returning a part(s), your return authorization number must be clearly marked on your return shipment label so that it can be properly tracked and processed.
